Vitamin D and Dental Health
Teeth are calcified tissue and require vitamin D for calcium handling, just like bone. Vitamin D also modulates the innate immune response in the periodontium and affects cathelicidin-mediated bacterial defence. Deficiency has been linked to caries, periodontal disease progression, and implant failure.
Caries in children
A 2013 meta-analysis of 24 pediatric studies (2,827 children) reported vitamin D supplementation was associated with a 47% reduction in dental caries incidence. Effect largest in children with baseline deficiency. Vitamin D contributes to formation of the primary and permanent dentition during the pre-eruption window, and to salivary immune function post-eruption.
Periodontitis
NHANES data (Dietrich 2004) found adults with 25(OH)D in the lowest quintile had more alveolar bone loss than those in the highest. Cross-sectional and longitudinal studies consistently link lower vitamin D with worse periodontal probing depths and attachment loss. Randomised trials of adjunctive vitamin D during periodontal treatment are small but favourable.
Dental implants
Retrospective studies suggest vitamin D deficiency (< 20 ng/mL) is associated with higher rates of early implant failure and impaired osseointegration. Some oral surgeons check and correct 25(OH)D pre-implant, particularly for grafting and sinus-lift cases where bone healing is critical.
Rickets, enamel, and the classic dental sign
Severe childhood vitamin D deficiency (rickets) causes enamel hypoplasia — visible as pitted, discoloured enamel, delayed tooth eruption, and increased caries susceptibility. The classic dental sign of nutritional rickets is a defective enamel band that indicates when the deficiency occurred developmentally.

- Standard vitamin D adequacy (25(OH)D ≥ 30 ng/mL) supports dental health.
- Fluoride, tooth brushing with fluoridated toothpaste, and dietary sugar restriction remain the primary caries interventions.
- Pregnant and breastfeeding mothers: adequate vitamin D contributes to healthy tooth development in the child.
- Pre-implant vitamin D correction is a reasonable pre-procedure step for deficient patients.
