Vitamin D and Vaccine Response
Vaccine effectiveness varies from person to person. Deficiencies in micronutrients that support immune function — vitamin D among them — have been examined as potentially reducing vaccine antibody response. Trial evidence for vitamin D supplementation before vaccination is limited but modestly favourable in deficient adults.
The biology
Vitamin D receptors are expressed on activated T-cells and B-cells. Calcitriol modulates the balance between Treg, Th1, Th2, and Th17 responses. Adequate vitamin D status supports both innate (cathelicidin) and adaptive (antibody production, T-cell function) immunity. Deficiency doesn't abolish vaccine response but may modestly reduce it in a subset of patients.
Flu vaccine evidence
Several observational studies find modestly lower post-vaccination antibody titres in vitamin D-deficient adults. The Chadha 2021 trial randomised older adults to vitamin D or placebo before influenza vaccination; the vitamin D group had modestly higher post-vaccination titres to some strains. The overall clinical significance is small.
COVID vaccine
Multiple observational studies examined vitamin D and mRNA COVID vaccine response. The most consistent finding: vitamin D-deficient adults had slightly lower peak antibody titres and slightly faster decline. Whether this reduced real-world protection is unclear — antibody titre correlates imperfectly with vaccine effectiveness.
Hepatitis B vaccine
Well-studied vaccine with clear correlates of protection (anti-HBs titre > 10 mIU/mL). Vitamin D deficiency associated with hepatitis B vaccine non-response in dialysis patients (Zitt 2012). Repletion before revaccination improved seroconversion in retrospective series.
Practical guidance
- Correct vitamin D deficiency for many reasons, one of which is optimal immune function.
- Do not delay a vaccine to correct vitamin D deficiency first — the delay is riskier than the modest immunogenicity effect.
- Chronic vaccine non-responders (hepatitis B, e.g., dialysis patients): checking and correcting vitamin D is a reasonable step alongside standard revaccination.
- Do not use vitamin D as a rationale for avoiding vaccines — it complements them, doesn't replace them.
