Vitamin D in Hot and Desert Climates

Some of the sunniest countries in the world have some of the highest vitamin D deficiency rates. Saudi Arabia, the UAE, Qatar, Kuwait, Iran, and North Africa — places with year-round intense UVB — routinely report 60–90% adult deficiency rates. The explanation is not the sun; it's what people do to avoid it.

The heat-avoidance pattern

In climates where midday summer temperatures exceed 40 °C (104 °F), spending time outdoors between 10 AM and 4 PM is impractical for most of the population. Life shifts indoors during peak UVB hours: air-conditioned homes, offices, malls, cars. Outdoor time happens early morning, evening, or after dark — outside the vitamin D-synthesising window.

Cultural dress

Conservative Islamic dress (thawb, abaya, hijab, niqab) covers most of the skin in public. Women wearing full coverage in public plus limited direct outdoor time at home have minimal effective sun exposure — often less than a Scandinavian woman in winter. Deficiency rates in adult women in some Gulf countries approach 95%.

Dietary patterns

Middle Eastern diets historically had modest vitamin D content — some fish in coastal regions, but fewer of the fortified dairy and cereal products that boost Western intake. Traditional dishes rich in vitamin D (oily fish, egg yolk) are present but not universal.

Health consequences

Rickets is documented in Middle Eastern children despite year-round sun availability. Osteomalacia and hypocalcaemic seizures are more common than in most Western populations. Bone health, muscle strength, and immune function all suffer. Regional health authorities have increasingly recognised the problem — Saudi Arabia, UAE, and Qatar have national vitamin D supplementation guidelines.

Fortification programs

Iran, Turkey, Saudi Arabia, and several Gulf states have implemented dairy and cereal fortification. Effects are modest so far — cultural dietary patterns and dress practices don't shift quickly, and fortification doses (typically 100–200 IU/serving) are small relative to the deficit.

Practical guidance for hot/desert climate residents

  1. Assume you are probably deficient — test 25(OH)D.
  2. Standard supplementation: 2,000 IU/day cholecalciferol year-round.
  3. Fully-veiled women: 2,000–4,000 IU/day.
  4. Correct severe deficiency with loading dose then maintenance (see loading dose page).
  5. Prioritise fortified foods when available.
  6. Brief early-morning outdoor time (7-9 AM in summer) is comfortable but produces limited UVB before sun climbs high enough.
